Steps to Enroll

1. Complete the AXA Inpatriate Medical - Enrollment Form ? 

2. Mail, fax or e-mail the Enrollment Form

3. Send the appropriate Plan premiums, as described in the Brochure or Master Application, by mail to Steven Cohen Insurance Agency Inc.

4. We will contact you for signature of the Master Application

5. The Master Policy will be issued in your Company's name

How to Claim for Medical Expenses

1. Download the Inpatriate Medical & Extended Health Claim Form.

2. Have your employees complete the claim form

3. Forward the completed claim form to AXA Assurances Inc. at one of the addresses listed on the form
